Description

In the event of a serious but non-life threatening condition, such as a laceration, fracture, fever, or vomiting, advise employees, students, or visitors to visit a local urgent care facility. For life-threatening medical emergencies, call 911.

Reporting Procedures

Visitor or Student (non-student worker) Incidents:

1. The supervisor, or other University staff, should complete the Student/Visitor Incident Report Form from the Insurance and Risk Management forms website. Do not use this form for employee incidents.
2. The form will route to the appropriate party within Environmental Health and Safety (EHS).
3. Notify HR by emailing delta-nscale-hr@ncsu.edu.

Student Workers and NC State University permanent employees:

1. If this is the first incident, fill out the General Incident Report Form. 
2. The form will route to the appropriate party within Environmental Health and Safety (EHS).
3. After completing the Google Form, inform HR at delta-ncscale-hr@ncsu.edu.
4. Review other reporting requirements outlined at the Environmental Health and Safety Accident Reporting website.
