Report an Accident
Description
In the event of a serious but non-life threatening condition, such as a laceration, fracture, fever, or vomiting, advise employees, students, or visitors to visit a local urgent care facility. For life-threatening medical emergencies, call 911.
Reporting Procedures
Visitor or Student (non-student worker) Incidents:
- The supervisor, or other University staff, should complete the Student/Visitor Incident Report Form from the Insurance and Risk Management forms website. Do not use this form for employee incidents.
- Please follow the instructions provided on the form and send the completed copy to delta-nscale-hr@ncsu.edu
Student Workers and NC State University permanent employees:
- If this is the first incident, fill out the Supervisor’s First Report of Incidence.
- Return the form to the listed parties at the top of the form and inform HR at delta-ncscale-hr@ncsu.edu.
